A person was killed while positioning a light pole on a concrete base using a crane. The light pole came into contact with overhead power lines.

A person was carrying out repairs to a four and a half tonne rigid truck, while the vehicle was running. During the operation, the truck clutch engaged and the vehicle travelled backwards approximately two and a half metres.
